Patient Forms
Having your forms completed before treatment helps us focus more of your time in the chair on care and conversation. Below are the forms we typically use for new and returning patients.
New patients are asked to complete a medical history form, patient registration and contact details, and consent forms related to treatment and privacy. Where possible, we will send these forms securely by email for you to complete online. If you prefer, you can download and print them, or arrive a few minutes early to complete them in the office.

From time to time, we may ask existing patients to update their medical history or contact details. This is especially important if your health status or medications have changed, you have moved or changed phone numbers or email, or your insurance coverage has changed. Keeping this information current helps us provide safe and appropriate care.
If you have difficulty reading, completing, or understanding any of our forms, please let us know. We are happy to review them with you, answer questions, or assist where needed.
